The programme’s crucial success criterion is to keep diabetics in the municipality within the zone, thereby creating healthier and more active citizens and making considerable financial savings for the municipality.
The programme is based on a results-based agreement between the municipality and Roche Diabetes Care, which ensures transparency in the costs related to treating diabetics, fewer expenses associated with complications and less costs associated with lost earnings due to inability to work. And because the municipality’s payment depends on the results achieved, there is no financial risk associated with agreeing to collaborate on the programme.
At the same time, the programme is easy to administer because Roche Diabetes Care takes care of the implementation and operation. It frees up resources, which could be used for i.e. cross-sectoral cooperation between municipalities, general practitioners and specialist doctors in the regions.
THE PRINCIPLE BEHIND
The principle behind the agreement between the municipality and Roche Diabetes Care is just as simple as it is innovative. The benchmark for the municipality’s payment to Roche Diabetes Care is index 100. Index 100 is reached when the treatment of the municipality’s citizens meets a satisfactory level agreed on by both parties. One definition could be that the patient performs 5 daily measurements, of which 70% are in the ’zone’, and that the diabetic indicates a satisfaction level of 4 with his/her treatment on a scale from 1-5.
Should the results of the treatment be higher than index 100, the municipality pays a higher price to Roche Diabetes Care and vice versa if the results are lower.
